Dr. Jason W. Cook is originally from St. Petersburg, Florida. He graduated for The University of Mississippi with a Bachelors degree in biology and a minor in chemistry in 1995. He obtained his Doctor of Optometry degree from UAB School of Optometry in 1999. His wife Tina works for the Alabama Institute for the Deaf and Blind in Talladega. They have two children, Kinley age 6 and Cason age 3.
